Chisago County provides ongoing case management for clients who receive home and community based services through waiver programs:

Long-Term Care Consultation

Long-term care consultation is a service that is available free of charge to any person looking to have their current and future needs assessed.

This screening process usually involves a team of both a nurse and a social worker and is usually performed in your home. The team will look at the services and resources you currently have in place and help you determine how your unmet needs can be met to help you stay in the community.

Nursing Home Relocation Services

Division staff provide transitional relocation services to clients who are in the nursing home who have a desire to return to the community.
