How to obtain a Certified Birth or Death Certificate (Minnesota only)

In Person- For births and deaths occurring in Minnesota, you may purchase a certified copy of a Certified Birth or Death Certificate by completing an application form and presenting valid government issued identification such as your driver's license. You will need select a reason that you are eligible to purchase the desired certificates.  Please Note: Most Death Certificates issued in the state from 1997 to present are available statewide. However, some Death Certificates must be obtained in the county the death was reported.

By Mail- Complete a Birth Certificate Application (PDF) or Death Certificate Application (PDF) You must sign the form in front of a Notary and mail the form with a check or money order payable to the Chisago County Recorder

Fees:

Birth Certificate

  • First certificate for each birth record ordered is $26
  • Additional certificates for the same record ordered at the same time is $19

Death Certificate

  • First certificate for each death record ordered is $13
  • Additional certificates for the same record ordered at the same time is $6

Payment of fees by check, cash or money order by mail or at the counter. Chisago County accepts credit or debit cards for payment of birth or death certificates at the counter only.
